Tax Season Checklist for Personal Income Taxes

As the year comes to a close, it is time to begin planning for the upcoming tax season. One 
step to prepare is to identify a personal checklist. This will allow you to provide your 
accountant with everything that they need to complete your return timely and accurately. 
Providing your complete information helps the process run smoothly and reduce the 
uncertainty and stress that comes with tax season.
Please keep in mind that the forms listed below will not be immediately available.

Tax Season Checklist for Personal Income Taxes: 

• Provided by Accountant:
  • Signed Engagement Letter
  • Completed Tax Organizer

• Provided by a Third Party:
  • Forms W-2
  • Forms 1099-INT and 1099-Div
  • Consolidated 1099’s
  • Forms 1099-R from Pensions, Annuities, 401k’s, and IRA’s
  • Form 1099-SSA, Social Security Benefit Statement
  • Forms K-1, if they are not prepared by your Accountant
  • Any other forms 1099 received by you (MISC, SA, NEC, Q, K, etc.) 
  • Forms W2-G and Gambling Loss Information
  • Form 1095-A from the Health Insurance Marketplace
  • Forms 1098 – Mortgage Interest Statement
  • Forms 1098T – Tuition Statement
  • Forms 1098E – Student Loan Interest Statement
  • Property Tax Bills

• Provided by You:
  • Records of Estimated Federal, State, and Local Tax Payments
  • Bank Account information for Direct Deposit/Direct Debit
  • Summary of Business, Farm, or Rental Income and Expenses
  • Summary of Medical Expenses
  • Summary of Charitable Contributions
  • Summary of retirement contributions, if not through W-2
  • Child care expenses 
  • 529 Plan Contributions
  • Education Expenses
  • Change in Dependent information, including birthdates and social security numbers

The IRS will no longer be sending refunds via check, please provide your preparer bank 
account information.
